常温机械灌注减轻猪心脏死亡供肝胆管损伤

【摘要】Objective To investigate the protective effects of normothermic machine perfusion on bile duct injury (IRI) in donation after cardiac death (DCD) porcine liver.Methods Twelve pigs were randomized into two groups (6 per group):the experimental group and the control group.In the control group,livers were subjected to 40-minute in situ warm ischemia,followed by subsequent 6-hour cold storage,mimicking DCD organ procurement and conventional organ transport.Livers in the experimental group underwent 40-minute in situ warm ischemia,but were subsequently perfused by normothermic machine for 6 h.The flow and pressure of hepatic artery (HA) and portal vein were monitored during machine perfusion in the experimental group.ALT and AST were measured every hour.Bile ducts were obtained for HE staining at the end of storage.Results The arterial and portal vein flows were stable in the experimental group during the entire machine perfusion.The levels of ALT and AST in the perfusate had a high increase in the first hour and then maintained in a stable state.The bile production gradually increased during machine perfusion and reached a peak in the third hour.The total bile production during machine perfusion was (27.6 + 0.6)
