抗纤灵冲剂对单侧输尿管梗阻模型大鼠
肾功能的影响

作者:张新志,吴锋,黄迪, 何立群, 董飞侠 【摘要】 目的观察抗纤灵冲剂对单侧输尿管梗阻(UUO)模型大鼠肾功能的影响。方法将120只SD大鼠随机分为正常组、假手术组、模型组、抗纤灵组、氯沙坦组。造模后7 d,14 d,21 d,28 d分批处死,腹主动脉采血,检测血肌酐(Scr)、尿素氮(BUN)。结果模型组大鼠Scr 、BUN显著升高,尿MA、β2-MG、NAG明显上升,与正常组及假手术组比较有统计学意义(P0.01);抗纤灵组大鼠Scr 、BUN显著下降,尿MA、β2-MG、NAG明显降低,与模型组比较有统计学意义(P0.01或P0.05),抗纤灵组与氯沙坦组比较,Scr明显降低,有统计学意义(P0.01),7d尿MA及14d β2-MG显著低于氯沙坦组。结论抗纤灵冲剂降低Scr 、BUN及尿MA、β2-MG、NAG,可能是通过舒张血管、调节管球反馈、改善肾小管间质纤维化及影响细胞因子、炎症介质的合成与分泌等多途径、多靶点地保护肾功能的。 【关键词】 UUO;抗纤灵冲剂;Scr;BUN;MA;β2-MG;NAG

Abstract:ObjectiveTo observe the effect of Kangxianling on the renal function in rats with unilateral ureteral obstruction.MethodsSD rats were randomly divided into normal group,sham operation group,model group,Kangxianling(KXL) group and Losartan group. After making model for 7,l4,21,28 d,the animals were killed in batch to get their blood through abdominal aorta,and the levels of Scr and BUN were determined.24 h-urine was collected and MA,β2-MG and NAG were detected.ResultsThe levels of Scr ,BUN in serum and MA,β2-MG,NAG in urine in model group were increased obviously,which were significantly different with normal group and sham-operation group (P0.01). The contents of Scr ,BUN in serum and MA,β2-MG,NAG in urine in KXL group decreased obviously,which were significantly different with model group (P0.01,P0.05).Compared with Losartan group,the level of Scr in serum was declined remarkably,MA on the seventh day and β2-MG on the forteenth day had the same tendency.ConclusionThrough expanding blood vessels,regulating tubuloglomerular feedback,lightening the fibrosis of renal interstitium,controlling synthesis and secretion of cytokine,KXL can protect the renal function and decrease the level of Scr ,BUN in serum and MA,β2-MG,NAG in urine.

慢性肾功能衰竭(chronic renal failure,CRF)严重危害人们的身体健康,国内外资料表明,CRF的发病率、患病率均明显上升[1,2]。虽然原发性肾病、肾血管病变和慢性间质肾炎这些病变进展到慢性肾功能衰竭的发病机制至今尚未完全阐明,但近10年来通过大量动物实验和临床研究发现,CRF的进展和预后不仅与肾小球损害有关,更与肾小管间质病变的严重程度密切相关[3],所以肾间质纤维化已成为CRF新的研究热点之一。流行病学调查发现,死于尿毒症的老人,30%以上存在梗阻病变[4]。以往的研究多集中于UUO对肾间质纤维化的影响,少有直接针对肾功能改变与RIF关系的研究。故而我们采用公认的研究梗阻原因造成肾间质纤维化的UUO模型 [5],以期为临床缓解肾间质纤维化提供一定的理论指导。所以我们拟通过检测血清Scr 、BUN、MA、尿β2-MG、NAG来观察抗纤灵冲剂对肾功能的干预作用。 1材料与仪器
1.1动物健康SPF级雄性SD大鼠120只,体质量(180±20)g,由上海中医药大学实验动物中心提供。分笼饲养于12 h光照,45%左右相对湿度的饲养笼中,自由饮食。
1.2药物及试剂抗纤灵冲剂水煎液:丹参15 g,制大黄15 g,桃仁12 g,当归12 g,牛膝9 g,由上海中医药大学附属曙光医院制剂室提供。水提浓缩,含生药2 g/ml。肌酐试剂盒、尿素氮试剂盒:均由南京建成生物工程研究所提供。
