多层螺旋CT血管成像诊断自发性蛛网膜下腔出血病因的价

【摘要】目的 探讨多层(16层)螺旋CT血管成像(computed tomography angiography,CTA)在自发性蛛网膜下腔出血的病因诊断中的价值.方法 回顾性分析98例自发性蛛网膜下腔出血的患者行16层CTA资料,成像扫描应用容积再现、最大密度投影、多平面重建等后处理技术显示血管,以明确病因.结果 16层CTA诊断12例正常,1例颈内动脉海绵窦瘘、1例硬脑膜动静脉瘘,9例动静脉畸形,75例动脉瘤,所有病例与DSA和/或手术对比证实,16层CTA漏诊1例,但其总的敏感性为98.7%,特异性为100%.结论 16层CTA诊断自发性蛛网膜下腔出血的病因具有较大的临床应用价值,可作为自发性蛛网膜下腔出血病因诊断的首选检查方法.%Objective To assess the value of 16-slicc CT angiography in diagnosis of etiology of spontaneous subarachnoid hemor-rhagc(SAH). Methods CTA data in 98 patients with SAH were summarized retrospectively. Cerebral vessels were displayed with volume rendering (VR) and maxiumum intensity projection(MIP) and multi-planar reformation(MPR) to find the causes of spontaneous SAH. Results CTA showed negative in 12 cases, internal carotid cavernous fistula in 1 casc,dural artcriovenous fistula in 1 case,intracranial artcriovenous malformation in 9 cases and intracranial ancurysm in 75
