代谢综合征及其组分与老年血尿酸水平的关系

【摘要】目的：探讨代谢综合征（MS）及其组分与老年血尿酸（SUA）水平的关系。方法采用整群随机抽样法选取任丘市年龄60～70岁的常住居民5010例为研究对象，进行面对面健康问卷调查、体格检查、实验室检查。依据2007年中国成人血脂异常防治指南中确定的MS诊断标准对研究对象进行诊断，并按照SUA水平进行四分位分组，探讨MS与SUA的关系。结果随着SUA水平的增加，男性所占比例逐渐增高，女性所占比例逐渐降低，MS、脑卒中或短暂性脑缺血发作（ TIA）、吸烟、过量饮酒、缺乏体力活动、抗血小板药物使用率逐渐增高，被动吸烟率逐渐降低，腰围、体质指数（BMI）、收缩压、舒张压、空腹血糖（FPG）、三酰甘油（TG）、胰岛素抵抗指数（HOMA-IR）逐渐增高，总胆固醇（TC）、高密度脂蛋白胆固醇（HDL-C）水平逐渐降低，差异均有统计学意义（P＜0.05）。男性血尿酸水平第1～4分位者 MS 患病率分别为15.77％、19.59％、23.98％和37.89％，女性分别为29.83％、32.58％、41.63％和59.47％。随着SUA水平的增加，男、女性MS患病率均呈增加趋势（ P＜0.05）。在调整了性别、年龄及MS其他组分后，Logistic回归分析结果显示，SUA水平是男性中心性肥胖、高TG血症的独立危险因素（ P＜0.01），是女性中心性肥胖、高血压、高TG血症、低HDL-C血症的独立危险因素（ P＜0.01）；与第1分位比较，男女性第3、4分位发生MS的危险增加（P＜0.01）。随着MS组分的增加，男女性SUA水平逐渐升高，HUA患病率亦逐渐
升高（P＜0.01）。结论 MS各组分与SUA水平密切相关，SUA水平是MS的重要组成部分，提示在MS预防和控制过程中，可将SUA水平作为MS预测及早期防治的一项指标。%Objective To investigate the correlation between serum uric acid ( SUA ) levels in the elderly and metabolic syndrome( MS ) and its components. Methods By using random cluster sampling method, we enrolled 5 010 permanent residents aged from 60 to 70 in Renqiu. Face-to-face questionnaire survey about health,physical examination and laboratory inspection were conducted on the subjects. According to the diagnostic criteria of MS in the Guidelines on Prevention and Treatment of Blood Lipid Abnormality in Chinese Adults,the subjects with MS were found out and grouped by the quartile of SUA level. Then the correlation between MS and SUA was investigated. Results With the increase of SUA,the proportion of male subjects increased,the proportion of female subjects decreased,the incidence of MS,stroke and TIA increased,the rate of smoking,excessive alcohol consumption and a lack of physical exercise increased,the usage ratio of antiplatelet drugs increased,the rate of passive smoking decreased,waistline,BMI,systolic pressure,diastolic pressure,FPG,TG and HOMA-IR increased,and the level of TC and HDL-C decreased,with all the differences significant(P<0. 05). The MS morbidity rates of male subjects in the four quartiles serum uric acid were 15. 77%,19. 59%,23. 98% and 37. 89% respectively,and that of the women subjects were 29.
83%,32. 58%,41. 63% and 59. 47% respectively. As the serum uric acid level increased, the morbidity rates of both male and female subjects increased. After adjusting for gender,age and other components of MS,the Logistic regression analyses showed that SUA level was the independent risk factor ( P<0. 01 ) for central obesity and high TG level in male subjects and the independent risk factor(P <0. 01)for central obesity,hypertension,high TG level and low LDL-C level in female subjects. Compared with the first quartile,the third and fourth quartiles were associated with higher( P<0. 01) risk in MS. The higher the quartile was,the higher the SUA level was for both male subjects and female subjects,so was the HUA morbidity rate ( P <0. 01 ). Conclusion SUA level is closely associated with MS components, and is an important element of MS. We suggest SUA level be taken as an indicator in MS predication and early precaution. 【总页数】6页(1995-2000)
