PDCA循环管理方法在住院药房调剂差错分析中的运用

【摘要】目的：采用PDCA循环管理方法(plan-do-check-action)降低住院药房调剂差错，以达到用药安全的目的。方法：按照PDCA循环的计划、实施、查核、处置程序，对我院住院药房2013年1月至2014年6月《药品调剂差错、事故登记本》记载的调剂差错，按季度进行回顾性分析汇总。分析其发生的原因，制定合理的整改措施，不断提高用药质量。结果：每季度的药品调剂差错逐渐减少，实际降幅达73%；调剂差错原因由开始时的客观因素占11.8%、主观因素占23.7%、易混淆因素占54.6%和其他因素占9.9%，到最后一个季度分别为48.8%、14.6%、22.0%和14.6%。结论：采用PDCA循环管理方法，调剂人员的整体业务水平明显提高，药品摆放更科学，逐步完善了调配工作各个环节的操作规程。今后要继续坚持PDCA循环的计划、实施、查核、处置程序，持续改进，有效预防药品调配差错，不断提高住院药房管理质量，确保患者安全用药。%Objective: To reduce hospital pharmacy dispensing errors by PDCA cycle management method (plan-do-check-action), so as to achieve the purpose of drug safety.Methods:According to the procedures of PDCA,the dispensing errors recorded \of Pharmacy Drug Dispensing Errors and Accidents\in our hospital from January of 2013 to June of 2014 were retrospectively analyzed by quarter. The causes were analyzed, corrective measures were formulated,
and the quality of the medicine was constantly improved. Results:Drug dispensing errors in each quarter were decreased, with an actual decline of 73%. At the ifrst quarter, the objective factors causing drug dispensing

factors
23.7%,confounding factors 54.6% and other factors 9.9%, which changed to 48.8%, 14.6%, 22.0% and 14.6% respectively at the last quarter.Conclusion: With the application of PDCA cycle management method, the personnel's overall business level has been improved signiifcantly; drugs were placed more scientiifcally; and the operating procedures for each deployment work were gradually improved. We should continue to adhere to the procedures of PDCA, so as to effectively prevent the drug dispensing errors, and constantly improve the quality of hospital pharmacy management, and ensure patient medication safety. 【总页数】5页(979-983)
