须 知

1.申请人应将本申请书和拟转换的产品认证证书复印件一式二份寄公安部消防产品合格评定中心, 第三份自己存档。

2.提供的产品认证证书应是1:1复印件，1997年、1998年、1999年和2000年取得的证书，须是获得认证机构2001年度监督确认后的复印件。 3.本申请表须用中、英文二种文字填写。

1. Applicant should send two filled copies of this Application Form and Products Certificate intended to be changed to China Certification Center for Fire Products (CCCF), and the third copy should be filed by the applicant.
