开腹与腹腔镜子宫肌瘤剔除术治疗多发子宫肌瘤效果比较

【摘要】Objective To investigate the effects of laparotomy and laparoscopic myomectomy on multiple uterine fibroids .Methods A total of 106 patients with multiple uterine fibroids treated in Department of Gynecology and Obstetrics of Yuci District People 's Hospital from July 2015 to March 2017 were selected and were divided into control group and observation group by random number table method , with 53 cases in each group .Patients in control group were treated by conventional open myomectomy , and patients in observation group were treated by laparoscopic myomectomy . The operation effect , immune function before and after operation and incidence of complications were compared between the two groups.Results There was no significant difference in operation time between the two groups ( P>0.05 ) .The amount of intraoperative bleeding , the time of ambulation and the hospital stay of observation group were all lower than those of control group , and the difference was statistically significant ( P<0.05).Before operation, there was no significant difference in the levels of IgA , IgG, IgM, CD3+and CD4+between the two groups (P>0.05).After operation, the level of IgA, IgG, IgM, CD3 +and CD4 +in observation group was
higher than that in control group , and the difference was statistically significant ( P<0.05 ) .The total incidence of complications in control group was higher than that in observation group, the difference was statistically significant (P<0.05).Conclusions Compared with traditional laparotomy , laparoscopic myomectomy can effectively improve effect of the operation , improve the humoral immunity and cellular immune function of the patients with multiple uterine fibroids , and reduce the incidence of complications .It is worthy of popularizing in clinical practice .%目的 探讨开腹与腹腔镜子宫肌瘤剔除术对多发子宫肌瘤患者的治疗效果.方法 选取2015年7月至2017年3月晋中市榆次区人民医院妇产科收治的多发子宫肌瘤患者106例,采用随机数字表法分为对照组和观察组,每组53例.对照组采用常规开腹子宫肌瘤剔除术治疗,观察组实施腹腔镜子宫肌瘤剔除术治疗,观察并比较两组手术效果、手术前后免疫功能及并发症发生率等指标.结果 对照组与观察组手术时间比较差异未见统计学意义(P>0.05),观察组术中出血量、下床活动时间及术后住院时间均低于对照组,差异有统计学意义(P<0.05).术前,两组患者的IgA、IgG、IgM、CD3+、CD4+水平比较差异未见统计学意义(P>0.05);术后,观察组的IgA、IgG、IgM、CD3+、CD4+水平均高于对照组,差异有统计学意义(P<0.05).对照组并发症总发生率高于观察组,差异有统计学意义(P<0.05).结论 与传统的开腹术治疗比较,对多发子宫肌瘤患者采用腹腔镜子宫肌瘤剔除术治疗,可有效提高手术效果,改善患者的体液免疫和细胞免疫功能,降低术后并发症的发生率,值得临床上推广使用. 【总页数】4页(96-98,102)
