探讨MRI动态增强血管成像对乳腺癌的诊断价值

【摘要】目的：探讨MRI动态增强血管成像中单侧乳腺血供增加对同侧乳腺癌的诊断价值。资料与方法对169例乳腺疾病患者行乳腺动态增强磁共振检查，在最大密度投影图像上进行血管计数，当血管长度≥3c m且最大直径≥2m m时，列入筛选合格范围，得到两侧的差异血管数目，即血管得分。两侧乳腺合格血管差值≥2个时认为单侧乳腺血供增加。分析乳腺单侧血供增加诊断同侧乳腺癌的灵敏度、特异度，以及与病灶大小的关联。结果以乳腺单侧血供增加作为同侧乳腺癌的诊断标准的灵敏度为65％，特异度为84％，阳性似然比(+LR)为4.06，阴性似然比(-LR)为0.42。T3肿瘤的平均血管评分显著高于T1和T2肿瘤。结论 MRI动态增强血管成像可以很好的显示双侧乳腺整体血供，单侧乳腺血供增加与同侧乳腺癌相关。%Objective This study was done to estimate the diagnostic performance of one-sided vascular prominence foripsilateral breast cancer. Materials and Methods A total of 169 patients underwent bilateral contrastenhanced magnetic resonance imaging.
