老年无保护左主干病变冠心病患者临床特点分析

【摘要】Objective To study the coronary artery pathological changes and clinical characteristics of the elderly with unprotected left main ( ULM) coronary artery disease. Methods PCI with drug-eluting stent was performed in 211 consecutive patients having ULMCA disease. The patients were divided into two groups,the elderly and the younger. The data from both clinical condition and coronary angiography were compared between the two groups. Results Compared with younger group,the rate of type 2 diabetes mellitus was much higher (P < 0. 05) in
the
elderly
group.
There
were
higher
rates
of
males,smoking,hyperlipidemia and glomerular filtration rate in the younger group. The level of blood pressure, blood high density lipoprotein-cholesterol, blood creatinine were significantly higher in the elderly group than in the younger group (P < 0. 05). Higher Syntax score and chronic occlusion lesion were more prevalent in the elderly patients (P< 0. 05). There was also higher number of diseased coronary arteries in the elderly patients (P = 0. 048). Compared with the younger patients,the rate of death was significantly higher in the elderly patients (P = 0. 021). Conclusion The elderly patients with ULMCA disease have
more severe clinical symptoms and coronary lesions. More attention should be paid to PCI for the elderly with ULMCA disease. One-year follow-up outcome after operation is ideal.%目的 观察老年无保护左主干(ULM)病变冠心病患者临床特点和冠状动脉病变情况.方法 选择行PCI的ULM病变冠心病患者211例,分为老年组(年龄≥60岁)111例和非老年组(年龄＜60岁)100例.比较2组患者的临床特点、冠状动脉病变程度及1年随访结果.结果 与非老年组比较,老年组患者年龄、糖尿病、收缩压、舒张压、HDL-C、血肌酐均明显升高(P＜0.05,P＜0.01);男性比例、高脂血症、TC、TG、LDL-C、肾小球滤过率、吸烟明显降低(P＜0.05,P＜0.01);冠状动脉病变Syntax积分高(P＜0.05),病变支数多(P＜0.05),慢性闭塞病变比例高(P＜0.05);老年组随访期间死亡事件发生率高(P＜0.05).结论 老年ULM病变患者临床症状复杂,冠状动脉病变程度重,介入手术应谨慎;术后随访临床效果良好. 【总页数】3页(601-603)
