多轴与单轴椎弓根螺钉治疗胸腰椎单节段骨折的对比研究

【摘要】Objective Comparatively study the clinical outcome of single thoracolumbar fractures with the treatment of multiaxial and uniaxial pedicle screws. Method 73 patients with single thoracolumbar fractures from 2009 to 2010 were retrospectively analyzed after inclusion-exclusion. They were divided into uniaxial pedicle screw group (49 cases ) and multiaxial pedicle screw group ( 24 cases ) . X-ray films were taken before operation, immediately after operation and at final follow-up. The kyphotic angle and the height of anterior and posterior edge of vertebral body were recorded and analyzed. Result There was no significant difference between two groups in the kyphotic angle and the height of anterior and posterior edge of vertebral body. Whilst within each group significant difference existed in the kyphotic angle and the height of anterior edge of vertebral body between the preoperative and the immediate postoperative, between the preoperative and the final follow-up. Conclusion Multiaxial pedicle screws used to treat single thoracolumbar fractures can achieve similar clinical results with uniaxial pedicle screws in correcting kyphosis, restoring the height of anterior edge of vertebral body and maintaining the good effect after operation.%
目的 对比研究单轴与多轴椎弓根螺钉治疗胸腰椎单节段骨折的临床效果.方法 回顾性分析2009 ～ 2010年治疗的胸腰椎骨折患者,经过纳入及排除标准,分为A组(单轴螺钉组)49例和B组(多轴螺钉组)24例,对术前、术后即刻及末次随访X线片的后凸角、椎体前缘高度、椎体后缘高度进行统计学分析.结果 A、B两组间在后凸角、椎体前缘高度、椎体后缘高度3个观察指标在术前、术后即刻和末次随访3个观察点无显著性差异.A、B各组中各观察指标两两相比,后凸角、椎体前缘高度术前与术后即刻、术前与末次随访差异有显著性,术后即刻与末次随访差异无显著性,椎体后缘高度差异无显著性.结论 多轴椎弓根螺钉治疗单节段胸腰椎骨折可以与单轴螺钉达到相似的临床效果,可以矫正脊柱后凸,恢复椎体前缘高度,并维持手术效果. 【总页数】3页(46-48)
