1例门诊儿童超说明书使用羟氯喹的药学服务

摘 要 目的：总结药师在前置审方时处理超说明书用药的药学服务及干预模式，为进一步优化前置审方工作流程提供参考。方法：审方药师针对医师超说明书用药的理由从法律、伦理、经济、技术、证据质量等多角度进行全面剖析并开展药学服务与干预。结果：较好地解决了临床诊疗实际需求与药品说明书更新滞后的矛盾，达成医药患三方接受的方案。結论：“以患者为中心”的全面分析、“疏堵相济”的通盘考虑是解决超说明书用药问题的有效模式，可以纳入前置审核工作流程。

ABSTRACT Objective： To summarize the pharmaceutical care and intervention mode of pharmacists in dealing with offlabel drug use in a child during the pre examination so as to provide reference for further optimizing the working process of the pre examination. Methods： The pharmacists comprehensively analyzed the reasons for doctors to off-label use drugs from law， ethics， economy， technology， quality of evidence and carried out pharmaceutical care and intervention. Results： The contradiction between the actual needs of clinical diagnosis and treatment and the lag of the update of drug instructions were solved and a threeway acceptance plan was
achieved. Conclusion： The comprehensive analysis of “taking patients as the center” and the overall consideration of “combining unblocking and blocking” are the effective modes to solve the problem of off-label drug use， which can be included in the pre audit workflow.
