实时三维超声心动图筛检冠心病的准确度评价

【摘要】目的 探讨考虑协变量影响的实时三维超声心动图左室前侧壁基底段收缩末期容积(ESV)筛检冠心病的诊断准确性.方法 收集2009年12月至2010年12月在晋城煤业集团总医院体检和就诊的疑似冠心病患者105例个人史等,并进行实验室及实时三维超声心动图等检查,以冠状动脉造影为\金标准\建立左心室前侧壁基底段收缩末期容积的受试者工作曲线(ROC)曲线回归模型.结果 体质量指数(BMI)值越大左心室前侧壁基底段ESV越大,在平衡了年龄等因素影响后,根据BMI分层提出,BMI＜19 kg/m2和＞24 kg/m2时,左室前侧壁基底段收缩末期容积筛检诊断准确性较高(曲线下面积分别为0.9505、0.8781).结论 考虑BMI的影响,进行实时三维超声心动图左室前侧壁基底段ESV曲线模型分析,可以提高对冠心病患者筛检的准确性.%Objective To investigate the diagnostic accuracy in screening of coronary heart disease (CHD) by using the real-time three-dimensional echocardiography (RT-3DE) which detects the end systolic volume (ESV) of basal anterolateral segment of left ventricle and takes into consideration the impacts of covariates. Methods We collected data such as medical history from 105 subjects with suspected coronary heart disease at health checkup or clinic visits between December 2009 and December 2010 in General Hospital of Jincheng Coal Industry Group. These subjects underwent
laboratory investigations and the RT-3DE. An ROC curve regression model for ESV of the left ventric -ular basal anterolateral segment was developed by using coronarography as the gold standard. Results The ESV of left ventricular basal anterolateral segment increased along with body mass index (BMI). After control for confounding factors (such as age) and stratification by BMI, we found considerably higher diagnostic accuracy in screening CHD by ESV of basal anterolateral segment of left ventricle when BMI was below 19 kg/m2 or more than 24 kg/m2, respectively [area under the curve (AUC) were 0.9095 and 0.8781]. Conclusion Accuracy of screening for CHD may be improved with RT-3DE ESV curve model of left ventricular basal anterolateral segment in combination of BMI. 【总页数】4页(1247-1250)
