股骨近端解剖型锁定钢板治疗老年股骨转子间骨折

【摘要】目的 评价股骨近端解剖型锁定钢板治疗老年股骨转子间骨折的疗效.方法 对39例老年股骨转子间患者采用股骨近端解剖型锁定钢板内固定,术后第2天开始行CPM机被动活动髋关节及膝关节的功能锻炼和直腿抬高及屈伸踝关节的主动锻炼.结果 39例均获随访,时间8～18个月,其中1例在随访5个月时因其它疾病死亡.38例骨折均骨性愈合,疗效评定按创伤髋评定标准(Sander 标准):优19例,良17例,可2例,优良率为92.31%.未发生断钉、脱钉、钢板松动、断板及髋内翻、关节僵直等并发症.结论 股骨近端解剖型锁定钢板对股骨转子间骨折具有内固定坚强、允许早期活动、预防关节僵直、有利于骨折愈合等优点,治疗老年股骨转子间骨折疗效较好.%Objective To evaluate the anatomic proximal femoral locking plate for treatment of senile intertrochanteric fractures.Methods 39 cases of elderly patients with intertrochanteric fractures were treated with proximal femoral anatomic locking plate fixation.2 d postoperative beginning, CPM was done to passive hip and knee function and range of motion exercises and straight leg raising initiative ankle exercise were trained.Results 39 patients were followed up for 8 to 18 months.1 case died in 5 month in the follow-up due to other diseases.38 fractures were healed, the efficacy was assessed by trauma hip evaluation criteria ( Sander standard ), which was excellent in
19 cases, good in 17.and fair in 2, and the excellent and good rate accounted for 92.31％.There was no broken nails, nail removal, plate loosening, broken plate and varus, joint stiffness and other complications.Conclusions Proximal femoral anatomic locking plate for intertrochanteric fractures with a strong fixation, allowing early mobilization, preventing joint stiffness contributes to fracture healing and others.The treatment of intertrochanteric fractures is good. 【总页数】2页(403-404)
