系及其临床意义

【摘要】目的 探讨上皮细胞黏附因子(EpCAM)在结直肠癌中的表达及其临床意义,探讨其表达与β-连环蛋白(β-catenin)表达的相关性.方法 应用免疫组织化学法检测120例结直肠癌组织和30例正常结直肠组织中EpCAM的表达,同时在120例结直肠癌组织中检测β-catenin的表达,结合临床病理特征及随访资料进行统计学分析.结果 EpCAM在结直肠癌组织中的高表达率约为60.8％,明显高于正常结直肠组织(6.7％,P＜0.01)；EpCAM在结直肠癌组织中的表达与TMN分期、淋巴结转移、远处转移及复发密切相关(P＜0.05). β-catenin在结直肠癌的高表达率约为70.0％,其表达与EpCAM的表达呈显著正相关(r=0.518,P＜0.01).生存分析表明结直肠癌患者的预后与EpCAM的表达、淋巴结转移、远处转移及复发密切相关(P＜0.05)；多因素分析表明EpCAM、TNM分期及淋巴结转移可作为结直肠癌患者的独立预后因素.结论 EpCAM在结直肠癌中的表达与β-catenin的表达呈明显正相关.%Objective To investigate the expression and clinical significance of epithelial cell adhesion molecule (EpCAM),and its correlation with β-catenin in colorectal cancer.Methods Immunohistochemical method was used to detect the EpCAM expression in 120 cases of colorectal cancer and 30 cases of normal colorectal tissues,and meanwhile detect the β-catenin expression in 120
cases of colorectal cancer tissues.The results combined with clinicopathological features and follow-up data were analyzed statistically.Results The rate of high EpCAM expression in colorectal carcinoma tissue (60.8％) was obviously higher than that in normal colorectal tissue (6.7％,P ＜ 0.01).The expression of EpCAM in colorectal carcinoma tissue was associated with TMN stages,lymph node metastasis,distant metastasis and recurrence (P ＜ 0.05).The rate of high β-catenin expression in colorectal carcinoma tissue was 70％,and significantly correlated with the expression of EpCAM (r =0.518,P ＜ 0.01).Survival analysis showed that the prognosis of patients with colorectal cancer was associated with EpCAM expression,lymph node metastasis,distant metastasis and recurrence (P ＜ 0.05).Multiple factor analysis showed that EpCAM,TNM stages and lymph node metastasis could be used as independent prognostic factors for colorectal cancer patients.Conclusion The EpCAM expression has a significantly positive correlation with the β-catenin expression in colorectal cancer. 【总页数】4页(1517-1519,封4)
