【摘要】 Objective To investigate the alteration of brachial-ankle pulse wave velocity (baPWV) in coronary artery disease (CAD) and observe the correlation between baPWV and coronary atherosclerosis. Methods baPWV was measured in a cross-sectional study involving 119 patients (78 males, 41females;mean age 56.6±12.7 years) who were undergoing coronary angiography. CAD was defined as≥50%diameter stenosis. The severity of coronary artery stenosis was qualified in a Gensini score. According to the three points bit interval of Gensini score,119 patients were divided into 3 subgroups:mild group (15 cases), moderate group (48 cases), severe group (56 cases). 132 healthy cases (70 males, 62females;mean age 53.7±11.4 years) who underwent annual health check up were chosen as control. General relevant factors such as age, gender, body mass index (BMI), smoking,blood pressure(BP),the total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), triglyceride (TG), fasting plasma glucose (FPG) and plasma urea acid(UA)were collected. Results baPWV remarkably increased in CAD patients compared with control subjects [(1960.1±283.1)cm/s

vs (1495.3±242.3) cm/s, P<0.01];baPWV

significantly rose with the severity of stenosed coronary vessels(mild 1593.7±282.4cm/s, 2379.6±372.2cm/s,

moderate P<0.01),

and

1997.2±322.3cm/s, they

both

were

severe positively

correlated(r=0.856, P<0.01).Logistic regression analysis demonstrated that baPWV is an independent risk factor for the presence of CAD(OR1.08, 95%CI:1.02,1.13). Conclusion baPWV is well correlated with atherosclerosis and has a certain value for evaluating the coronary artery early sclerosis. baPWV has potential as a new simple and noninvasive marker for prediction of CAD and screening the general population.% 目的研究不同严重程度的冠心病患者臂-踝脉搏波传导速度(brachial-ankle pulse wave velocity,baPWV),探讨其与冠状动脉粥样硬化的关系.方法这项横断面研究连续纳入119名[男性78人,女性41人,平均年龄(56.6±12.7)岁]经冠状动脉造影诊断为冠心病的患者(冠状动脉狭窄程度≥50%),并对冠状动脉的狭窄程度进行定量Gensini评分,根据Gensini评分的三分位间距将患者分3个亚组,轻度组(15例)、中度组(48例)、重度组(56例).同期选择132名健康体检者[男性70人,女性62人,平均年龄(53.7±11.4)岁]作为对照.分别采集每一位受试者的臂踝脉搏波传导速度以及一般资料,如年龄、性别、体质指数(BMI)、吸烟情况、血压、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C),高密度脂蛋白胆固醇(HDL-C)、三酰甘油(TG)、空腹血糖(FPG)、血尿酸(UA).结果与对照组比较,冠心病组baPWV明显增高[(1960.1±283.1) cm/s vs (1495.3±242.3) cm/s,P<0.01];单因素方差分析显示,冠状动脉狭窄程度越严重, baPW值越高[轻度(1593.7±282.4)cm/s,中度(1997.2±322.3)cm/s,重度

(2379.6±372.2)cm/s,P<0.01].多元线性回归分析显示, baPWV与冠状动脉狭窄程度的评分呈明显正相关(r =0.856,P<0.01).Logistic回归分析显示baPWV是心血管疾病的一项显著的独立相关危险因素(OR1.08,95%CI:1.02,1.13).结论 baPWV与动脉硬化具有良好的相关性,其对了解冠状动脉早期硬化及其程度有一定的价值.baPWV可以作为临床上一项简单、无创性的筛选心血管疾病高危人群的指标.
