海绵窦区脑膜瘤的手术疗效分析

【摘要】Objective To study the indications,methods and skills of microsurgical resection for cavernous sinus meningiomas and its effects on musculi oculi.Method 25 cases of cavernous sinus meningiomas were analyzed retrospectively from July 2000 to July 2006.Biglan grading were used to analyze the variations of musculi oculi function at the points of before operation.first week after operation and follow -up,Results The tumors were totally removed in 4 cases,subtotally removed in 14 cases,and partically removed in 7 cases.New neurological deficits were noted in 19 cases after operations.Ptosis was abserved in 18 cases.22 cases received fellow-up research by means of 46.2 months.The total recurrence rate was 12%.The ratio of excellence grade plus good grade in Biglan grading were 64%,52%and 68%respectively before

periods.Conclusions Though total resection was difficult to peform in cavernous sinus meningiomas,rational microsurgical resection is an effective treatment.and it is helpful to improve the prognosis of the effcted musculi oculi.%目的 探讨显微手术治疗海绵窦区脑膜瘤的疗效,评价手术对眼肌功能预后的影响.方法 总结经外科手术切除的25例海绵窦区脑膜瘤患
者的临床资料.分别在术前、术后1周以及随访期间应用Biglan眼肌功能分级对患者进行眼肌功能评定.结果 肿瘤全切4例,近伞切除14例,大部切除7例;术后19例出现了新的脑神经功能缺失症状,术后上睑下垂18例;22例得到随访,平均随访期为46.2个月,总体复发率为12%;术前、术后1周、随访期间Biglan眼肌功能分级中优秀+良好的比率分别为64%、52%和68%.结论 海绵窦区脑膜瘤全切困难,规范的显微外科手术是有效地治疗措施,有助于改善受累眼肌的远期预后.
