水液缺乏性干眼患者角结膜结构共焦显微镜观察

【摘要】Objective To evaluate Keratoconjunetival morphological changes of subjects with aqueous tear deficiency using laser confocal microscope.Methods 58 eyes of 45 patients who were diagnosed as aqueous tear deficiency,were chosen as the study group of dry eye.Other 90 eyes of 45 control subjects were included in the study,the images of superior,inferior limbus,central cornea and temporal conjunctiva

HRTII/RCM

laser

confoeal

microscope.Results The surface epithelial cell density was 706±261cells /mm2 at the superior iimbus,689±238cells /mm2 at the inferior limbus and 812+ 256 cells /mm2 at the central cornea.Compared with control group(847±302cells/mm2 at the superior limbus,803± 293cells / mm2 at the inferior iimbus and 1124± 290cells /mm2 at the central cornea),There was statistical difference accordingly between the two group(P<0.05). The number and tortuosity of sub-basal nerves at central cornea was 8.4±1.4b/p and 2.1±1.1 in control group and 9.1±1.6b/p and 2.7~ 1.3 in dry eye,There was statistical difference accordingly between the two group(P<0.05). The morphology of langerhans cell was irregularity.The processes of langerhans cell in dry

eye were longer and more than that in control group.Epithelial thickness.corneal thickness and temporal conjunctival epithelial cells density were smaller in dry eye cases than that were in healthy subjects.Conclusions Aqueous tear deficiency can affect the structure of the coma.Laser confocal microscopy is valuable for basic research and clinical diagnosis of corneal disease.%目的 探讨激光共焦显微镜下水液缺乏性干眼患者角结膜结构的形态学特征.方法 应用激光共焦显微镜(HRTⅡ-RCM)对45例(58只眼)干眼患者及45名(90只眼)正常人的中央角膜,上下方角膜缘及颞侧结膜进行检查分析.结果 正常眼角膜中央、上下方角膜缘表层上皮细胞个数平均密度分别为(1124±290)个/mm2、(847±302)个/mm2和(803±293)个/mm2;干眼患者分别为(812±256)个/mm2,(706±261)个/mm2,(689±238)个/mm2,两两比较差异具有统计学意义(P<0.05).干眼患者与正常眼中央角膜上皮基底层下神经纤维每幅的数量分别为(9.1±1.6)支和(8.4±1.4)支(P<0.05),弯曲度为(2.7±1.3)度和(2.1±1.1)度(P<0.05).干眼患者角膜上皮下Langerhans细胞形态旱树支状,其突起较正常眼数量多且长度较长.干眼患者角膜上皮厚度、角膜厚度及颞侧结膜上皮细胞平均密度均低于正常(P<0.05).结论 水液缺乏性干眼对角结膜结构有重要影响,激光共焦显微镜具有广阔的基础研究和临床应用前景.
