【摘要】Objective To assess the value of color Doppler sonography (CDS) in detecting perforator vessels in the donor area before anterolateral thigh flap harvest and provide guidelines for the clinical application in pediatric population.Methods From January 2013 to September 2014,sixteen children with soft tissue defects underwent anterolateral thigh flap transfer.The flaps were used to cover soft tissue defects of the foot in 6 cases,of the leg in 2 cases,and of the hand and forearm in 8 cases.The age of the patients ranged from 16 months to 12 years.The defect areas ranged from 6.0 cm × 9.5 cm to 8.0 cm × 14.0 cm.CDS was used preoperatively to locate the perforators.The accuracy of the anterolateral thigh perforator location was checked during the surgery.Results The predominant perforators in the anterolateral thigh flaps were accurately located by preoperative CDS in all 16 cases and adopted intraoperatively.In 2 cases more dominant perforators were found intraoperatively,which were included in the flaps.The rate of coincidence between the preoperative and intraoperative localization of perforators was 87.5 ％.All flaps survived uneventfully.The appearance
and functional results were satisfactory after 3 to 15 months' follow-up.The donor site wound was primarily closed in 14 cases and covered with partial skin graft in 2 cases.Conclusion Preoperative CDS can accurately locate perforators of the anterolateral thigh flap in pediatric cases and therefore should be widely used.%目的 探讨彩色多普勒超声(color Doppler sonography,CDS)在小儿股前外侧皮瓣穿支血管术前定位中的应用价值,为临床推广应用提供参考依据.方法 2013年1月至2014年9月对我院16例拟行股前外侧皮瓣手术患儿,修复小儿足部软组织缺损6例、小腿部2例、手与前臂8例.年龄16个月至12岁,缺损面积6.0 cm×9.5 cm～ 8.0 cm× 14.0 cm.术中观察CDS术前定位旋股外侧动脉穿支的准确性.结果 16例患儿股前外侧皮瓣优势穿支定位准确且被术中采用,2例术中另外发现更优势穿支并采用,CDS术前定位符合率87.5％;术后所有皮瓣均存活,经3～15个月随访皮瓣外形和质地良好;供区14例直接关闭缝合,2例通过部分植皮.结论 彩色多普勒超声是小儿股前外侧皮瓣穿支术前定位较为理想的定位方法,值得在临床中推广应用. 【总页数】3页(257-259)
