【摘要】objective:to explore the effect of hospital-community-family-based
management
on
risk
factors
in
diabetes
mel
itus.Methods:Standardized , systematic of health education and health living style intervention on diabetes mel itus risk factors patients and their family members.Results:After the intervention of the patients with diabetes mel itus risk factors , there was significant difference (p<0.05)with control group in normal fasting blood glucose, triglycerides, Total plasma cholesterol(TC), high density lipoprotein (HDL) and low density lipoprotein(LDL). Conclusion:Systematic ,standardized of health education intervention with hospital-community-family-based on diabetes mel itus risk factors patients and their family members, can improve the normal fasting blood glucose, triglycerides, Total plasma cholesterol(TC), high density lipoprotein (HDL) and low density lipoprotein(LDL), to reduce the development of diabetes has important significance on the prevention and control.%目的：探讨医院社区家庭联合管理糖尿病高危因素患者的效果。方法：对糖尿病高危因素患者及家属进行规范、系统的健康教育和健康生活方式的干预。结果：干预后的糖尿病高危因素患者空腹血糖、甘油三酯、总胆固醇、高密度脂蛋白、低密度脂蛋白与对照组
