代谢综合征组分与非酒精性脂肪性肝病的关系

【摘要】目的:探讨代谢综合征( MS)组分与非酒精性脂肪性肝病( NAFLD)的关系,为MS不同组分患者防治NAFLD提供参考。方法选择2014年6月—2014年12月在天津医科大学总医院接受治疗的符合MS组分之一及以上的患者344例,将患者按照是否符合MS诊断标准,分为MS组和非MS组;同时,又按照MS组分数目进行分组,满足其中1项组分为组1,满足2项组分为组2,满足3项组分为组3,全部满足为组4。抽取患者空腹静脉血,检测空腹血糖( FPG)、糖化血红蛋白( HbA1c )、总胆固醇( TC)、三酰甘油( TG)、高密度脂蛋白胆固醇( HDL-C)、血尿酸( SUA)和24 h微量清蛋白尿( MAU)。患者均行上腹部肝脏超声检查,判定是否患有NAFLD。结果两组性别、年龄、FPG及 HbA1c比较,差异均无统计学意义( P >0.05)。MS 组体质指数( BMI)、收缩压( SBP)、舒张压(DBP)、TC、TG、SUA、MAU、NAFLD患病率均高于非MS组,HDL-C低于非MS组(P<0.05)。随着代谢异常组分的增多,BMI、SBP、DBP、TG、SUA、NAFLD患病率逐渐增高,HDL-C水平逐渐下降(P<0.01)。Logistic回归分析结果显示,BMI和TG是NAFLD发生的独立危险因素( P<0.05)。结论患者BMI、TG和MS组分数目与NAFLD关系密切,监测这些指标有助于早期发现NAFLD高危人群。%Objective To investigate the relationship between metabolic syndrome ( MS ) components and non -alcoholic fatty liver

disease( NAFLD) in order to provide references for the prevention of NAFLD in patients with different MS components. Methods A total of 344 patients who accorded with at least one of the MS components and received treatment in the General Hospital of Tianjin Medical University from June 2014 to December 2014 were enrolled and were divided into MS group and non MS group according to MS diagnostic criteria. The subjects were also divided into group 1,group 2,group 3 and group 4,according to the number of MS components they accorded with. The fasting venous blood of the subjects was sampled, and then FPG, HbA1c , TC, TG, HDL-C, SUA and 24 h MAU were tested. The subjects all received liver ultrasonic inspection in the epigastric section to determine whether they had NAFLD. Results The two groups were not significantly different(P>0. 05)in gender,age,FPG and HbA1c. The MS group was higher(P<0. 05)in BMI,SBP,DBP,TC,TG, SUA,MAU and the prevalence of NAFLD and was lower(P<0. 05)in HDL-C than the non-MS group. With the increase of MS components,BMI,SBP,DBP,TG,SUA and the prevalence of NAFLD increased,and level of HDL-C decreased( P<0. 01). The Logistic regression analyses showed that BMI and TG were independent risk factors(P<0. 05)for the occurrence of NAFLD. Conclusion

The

relationship

between

BMI,TG,number

of

MS

components and NAFLD is close. The monitoring on the three indicators could contribute to the early detection of NAFLD.
