病案首页主要诊断选择缺陷分析

[摘要] 目的 通过对某医院2017年出院患者的病案首页主要诊断存在的缺陷情况进行分析，并提出对策，为医院的医疗质量管理和医疗信息提供依据。方法 根据国际疾病分类原则，对42 124份出院病案首页主要诊断进行审核。结果 缺陷的内容主要是疾病诊断名称书写不规范、主要诊断选择错误。阐述了选择主要诊断的方法和技巧。结论 提出加强对医师进行国际疾病分类知识的培训和指导，有助于临床医师正确书写主要诊断，提高ICD编码的准确性，确保医疗信息的统计质量。

Methods 42 124 pieces of discharged medical record home pages were reviewed according to the classification principles of international diseases. Results The major defect contents were the non-standardized disease diagnosis title writing and incorrect selection of major diagnosis， and the paper elaborates the major diagnosis method and skill. Conclusion We should enhance the training and guidance of international disease classification knowledge of physicians thus helping the clinical physicians accurately write the major diagnosis， improving the accuracy of ICD code and ensuring the statistical quality of medical information. 病案首页是临床诊疗信息的精华，是卫生信息统计、医疗管理、临床医学研究的重要原始数据来源，而主要诊断是最根本的，一切诊断、治疗、护理、检查都围绕主要诊断进行，它直接反映医院的医疗质量和管理水平。随着诊断相关组（DRGs）及預定额付费制（PPS）的不断推进，以及根据诊断相关组结果，对医院医疗绩效工作评价的开展，主要诊断的选择与编码对医院付费、医疗绩效工作评价、医院管理、保险赔付的影响显得尤为重要。 1 资料与方法
资料来源于某院病案管理科，由病案管理科疾病编码人员，根据ICD—10的编码规则，对2017年出院患者的病案首页42 124份，医师填写的主要诊断审核时发现缺陷情况进行分析。
