X线根尖片与CBCT在牙根纵裂诊断中的临床价值

【摘要】Objective To discuss the clinical value of periapical radiography and cone beam computed tomography(CBCT) in the diagnosis of vertical root fracture. Methods 60 patients suspected of vertical root fracture were given periapical radiography and CBCT, then confirmed by surgical treatment, compare the sensitivity, specificity, positive predictive value, diagnose accordance rate and correct indices between the two inspection methods. Results 64 teeth was diagnosed as vertical root fracture.56 teeth of buccolingual vertical root fracture (87.50％), 8 teeth of near-far centrad vertical root fracture (12.50％). Inspection result of periapical radiography: 56 teeth were diagnosed as vertical root fracture, 12 teeth eliminated, inspection result of CBCT: 63 teeth were diagnosed as vertical root fracture, 5 teeth eliminated. The sensitivity, specificity, diagnose accordance rate and correct indices of CBCT were significantly higher than those of periapical radiography (P ＜0.05 or 0.01). Conclusion In the clinical diagnosis of vertical root fracture, CBCT has higher sensitivity, specificity and diagnose accordance rate, it also has higher accuracy, and should be taken as routine examination to reduce the misdiagnose and missed diagnosis.%
